It deserves a negative rating. If you hate your family, put them here. Roaches in the resident's bathrooms and black mold. I cry thinking about how awful the residents were treated. The turnover rate is one of the worst I have ever witnessed. All nursing assistants and nurses are overworked and understaffed so they often cannot or do not have the time or ability to provide the care the residents need. Residents go 2-3 weeks without bathing at a time and getting their teeth brushed? dream on. A majority of the staff is great and sincerely tries there best, but the ones who don't? It's sad. They will leave residents who cannot walk, alone in the showers by themselves. I hope this place gets shut down.

I wish I could give negative stars!!!!
This place is awful! One it smells like urine the second you walk in. The place is so dirty. My father was a patient there and had to leave via ambulance after ONE week. The lack of care was disgusting. When he got to the ER the night he left this facility his PICC line bandage wasn’t changed they just put surgical tape on top, and his clamps to his PICC lines were left open. The most critical thing on his body!!! One of his nurses Janet was so awful just rude didn’t care about anything, didn’t give my father his insulin for 4 hrs after his blood sugar read 390. And she said oh sorry I forgot.
One of my father’s other nurses couldn’t tell the difference between a ostomy stoma and a bell button. Can’t make this stuff up!! Please DO NOT SEND YOUR FAMILY HERE!

Great rehabilitation team with ongoing goals heard by the rehab staff and helpfully assisted by great team workers that are best suited for their positions. Highly …highly…. Recommended facility if you’re looking to get back on your feet or have your life back on track. Many suitors in the rehab department could be mentioned but, to me I must say that the best people in my opinion are to be Katie the head of therapy, Bob who runs COTA, Dominic who is the greatest PTA to work with and Kevin who is the a wonderful RNA that seems to help in every field wherever you might be. Not playing around but this is a journey that I will not forget about because of everyone I have been lucky enough to encounter

I have received top notch care at Phoenix Mountain, especially by the activities and therapy departments. Bob and Dom in the therapy department both have been there from the beginning, encouraging me and helping me build my strength back up and truly have the best interest for me when they provide care. The entire staff have been there for me, for which I am immensely grateful for.

Phoenix mountain post-acute.I am slightly disappointed in this building.I've worked this building through registry a couple of times and I thought this place would be a good fit to get my mother back on her feet or somewhere back tounfortunately my mother's health condition speech and so forth. Has?

Been there so I am taking my mother home.There's no better place.You have a few nurses.Have you a few cnas That care, but the main person that I can truly say that's on their job is Corey. Taste manager and Julio on the 400 hall Our family Appreciate what care has been done for my mother?
But she will be better off at home.

Worst place ever. Same complaints as the other 1-star reviews listed here. understaffed and overworked, poor patient care. I had a family member there, 85 year old woman - she got terrible care. didn't give her pain meds for 2 hours+ past the time due. she was in pain, crying, desperate and couldn't think. 3 people intervened and spoke to nurses/CNAs for days, they would not listen or help, rude, bad attitude, arguing, and didn't care at all!
her commode was not emptied often enuf, despite her and us asking. during night she used it and it overflowed onto the floor because of lack of emptying - disgusting.
gave her wrong food for 10 day stay - she has Celiac disease and they gave gluten food every meal, she sent it back but couldn't get right food, hungry, ate snacks rather than meals, later got very sick for days from their mistake.
they don't answer call button at all or could take an hour. Dirty sheets not changed. place smells of urine and other when you walk in.

My father was a patient at the Phoenix Mountain Post -Acute for about a week. All of the staff members at the facility were excellent. They were friendly and supportive of my father. The facility was clean but very outdated. It needs to be remodeled. Also, they say spacious rooms on their website but they are more like a closet. My father had to share a room with someone. The staff is wonderful but the facility itself is out of date. The rooms are too small for 2 people. Needs improvement

I was there 3 weeks. I will NEVER return to that facility!
Aside from 3 to 4 caregive nurses that cared. The place was not so concerned. The food you would not feed your pets because they would refuse!
When hitting call button not all but many took 30 to 45 mins to respond. 2 roommates a nightmare. One from amplified snoring volume so no sleep. The other had dimensia and speaking to imaginary people at full volume all night. List items of mine in room changes.. They don't talk to each other about patient needs I'll never recommend or go back there !
