Forever grateful to the team at Vineyard. When my grandmother was transferred there, we didn't think she'd be able to go home again, but the therapy team was absolutely amazing In just a few weeks, she was up and walking, and we were able to take her home where she truly wanted to be. Later, my 82-year-old father-in-law broke his hip and I knew exactly where he needed to be. It was a hard recovery, but the therapy team, CNA's, and nurses were incredible. So much gratitude for these hardworking people who truly care about their patients. Thank you 🙏.

Forever grateful to the team at Vineyard. When my grandmother was transferred there we didn't think she'd be able to go home again but the therapy team was absolutely amazing! My husband has been at Vineyard Post Acute since December 2023. His experience has been great. He is treated SO well by all the great staff there from top to bottom, and he has had the BEST care by the Doctors & Nurses. He tells me the meals are always incredible! The administrative staff there has been great, and in particular Gabby has been BEYOND helpful to us. The physical therapy there is the best, and the staff in that department is off the charts! (pun intended) So far it has been a very positive experience for my husband Tom.

This was the worst experience I ever had to go through. I had a grilled cheese sandwich for a meal with some kind of red collared liquid “juice”. The 4 person room was dirty with a small sink/bathroom if that’s what you could call it. I had a cell phone but there was no cell service… I could go on and on but I hope you get my experience. The living room was lovely, clean, fresh but go beyond and behind the curtain you’ll see a totally different environment it’s. a 00 for my rating.

My nephew has been here for about 3 years. I/ we feel that he receives good quality Care here.The staff and CNA are always nice.They always Greet us with a smile, always pleasant ask us who we are seeing. When my Nephew needs help or assistance.They come to his bed site and help him with his needs, or give him his meds. When he comes down ill they assist medical care for him. They let us know in regards to his health if something changes like If they cannot help him with his medical care they send him over to the hospital were he could get his quality medical attention taking care of. They give quarterly phone meetings to his family in regards to his care in his daily/ weekly, in his daily health, in his medical update at the facility. What's good for him and what's not going well for him.My nephew has great roommate that gets along together.

My mother was transferred from the hospital to Vinyard Post Acute.
She was OK on Thursday, but on Friday she was in pain. By Saturday, we tried to get her a medical transport back to the hospital because her pain had increased and she was in great distress.
Never ask for a “medical transport”, they will just ignore you. They stated her BP was normal, no fever, and no temp, so she was fine! By early the next morning, we called 911 and got her out of there. She was taken to Petaluma Valley Hospital and they found a GI blockage. Her story ended quite quickly after that.
I felt that the care at Vineyard Post Acute to be lacking. They did not care if a patient was in pain as long as their vitals were ok. Please don’t take your loved ones to Vineyard. I don’t know if it’s under-staffed, or just insensitive, but don’t take your people there.

The attendant would not call 911 for my roommate Connie, whose daughter called for her instead. She kept telling him this is a place of death and pain. It was clear she was in distress. He said to be patient, to wait for the nurse on the other side of the facility. She barely got out of there alive! He came back to see that her bed was empty. He angrily “accused” me of calling 911. She was in critical condition, bleeding internally, staying in the hospital for 2 months. I had to leave Vineyards right after that incident.
It was so traumatic for her and her daughters. We stay in touch because we care about her.
I had two bad things happen with 2 CNA’s. They pale in comparison to Connie. She is the sweetest woman on earth. I’m so glad she made it.

My mom was just discharged from Vineyard after 6 weeks of post acute care. We were very pleased with the care she received and the cleanliness of the facility. The staff were outstanding and very attentive to her needs. The front desk staff were very nice as well. We dealt with OT, PT, Nurses, CNA's and never once had a bad experience. Everyone was kind. We had family there everyday so we saw everything and have nothing but good things to say.
I was only involved with the post acute care so that is what my review is for.
