To see the smiles on the residents faces it fills me with warmth. Arriving to such a welcoming environment. The staff is exceptional and empathic towards each patient making them feel like one of their own. Keeping family involved as much as they can. I absolutely love all the indoor and outdoor activities the resident have to choose from helps to keep them feeling alive and apart of the family this facility offers. I just know when it's my time to enjoy senior living Southern Trace is number one on my list to call my next home away from home.

I have worked here for the past year and have enjoyed it. I think we have an amazing therapy team and hands down an awesome charge nurse for our hall. Over all I have experienced great team work and love working with these people. Administration is great too. Nancy truly loves her residents and staff and it shows. She always has time to listen to anything that comes up and handles things in a very professional manner.

Southern Trace has been a great place to work! Everyone works as a team and the elders are cared for like family. There’s always something for them to do, and plenty of opportunities for the staff to get involved with them on more than just a patient care level. Employees from every department are encouraged to take part in activities and have some fun with the elders too. The nurses are top notch and the CNAs are amazing too! I’ve worked in a few facilities, but none of them compare to Southern Trace!

I have worked at Southern Trace for 18 years. Many of the department managers have a long tenure with the facility. There are also CNAs that have worked 20+years. Southern Trace has been voted Best of the Best Nursing Home is Saline County again in 2024. I invite families looking for a home for their loved ones to include Southern Trace in your tours. If I could give this place NEGATIVE 100 stars I would. My mother was neglected to the point I had her transported to the hospital to get the proper end of life care she deserved. She should have been put in hospice but because the administrator was not there on the weekend she did not get the proper medications and had to suffer. Sunday came and she was allowed antivan which was suppose to be every 2 hours but she had to wait 7 HOURS to get ONE dose and she did Not get another dose until after midnight Monday morning. 7:30am Monday morning my mothers hospice contract was still not signed as it turns out the administrator was on vacation. Apparently no one goes on hospice over a weekend? I was promised by the ADON that she would go on palliative care and get the proper medications and would not have to go through a 3rd party to get the proper medications… she required medication for her secretions as she could not clear her throat in her fragile state. I was told we would get the order in right away and by 3pm STILL NO MEDICATION as they were waiting on the nurse practitioner to come in…This is neglect!!!! To top it off the staff put a RUBBER BAND in my mothers fine thin hair, poop in the shower that had been there for two weeks- we know it was two weeks because a worker started screaming at the foot of my mothers bed “I told you two weeks ago that was in there.” My mother was given not 1 but 2 broken oxygen machines so she wasn’t receiving the proper oxygen.
This was the WORST facility I have been to. Luckily we were able to transport my mother to Saline Memorial where she was given the proper end of life care and was able to leave this Earth with her dignity and her daughter by her side and all the proper medications keeping her comfortable and pain free. Do NOT send your loved ones to this hell hole.
I will praise Kiana the speech therapist who treated my mom with love, dignity, and compassion. Ashley the night nurse she had on 10/ 27 and Dash her CNA were two of the most loving and compassionate people I’ve ever met.
The ADON, DON, and administration staff should be ashamed of themselves.

I was denied a sponsorship for CNA classes and position because I decided to read the reviews on this facility. After doing research through fellow nurses who knows and encountered this place didn't have anything good to say about the current nursing staff nor the administration. Current nursing staff is Valerie and administration who made the decision was NANCY. I took my TB skin test and drug test and once I came back to have my TB test read that's when I was to that they would no longer sponsor my CNA classes because I inquired about the reviews and asked if there were anything I needed to be concerned with. Valerie admittedly told me, YOU DON'T READ REVIEWS OF A JOB YOU'RE SEEKING OUT AND THEY NO LONGER WANYED TO SPONSOR ME. That's a definite and the reviews as they speak about the care of the residents, I saw just walking through some things I was side eyeing. I don't they are really interested in hiring good people who feel they can make change in the medical field because we are definite mandated to where you see something you say something. This facility owners are hidden because their practices with having upwards of 34 facilities are just basically taken money and giving piss poor care for these people. I've been in the dental sector of the medical field and going for a career change to better lend my services to our aging community. I advocate for my current patients to get the best dental care possible and WILL do that more through nursing. My grandmother died from the lack of care in a nursing home so my heart has always been for the care of my seasoned patients. The CNA I saw had on muddy shows. Another clothing was not clean and had spots on it, not from work. The entrance was nice. I heard a lot of confusion of talks with other staff not knowing who was supposed to do what and where certain staff was. Only because I was left at the front for an extensive amount of time waiting. Understanding things comes first before I was there, but just to hear it was baffling. Would you want confused staff members caring for your loved one when maybe a important meds is to be given, or maybe they don't give it all. I would definitely think twice before placing a loved one here or simply working for them, because if it seems you got a brain and some type of competence then your their worst enemy. NANCY makes this facility a lot of money I heard and they seem to like her shady practices as it brings the facility money,,,ALL PROFIT. VALERIE the nurse is rude and didn't show up for work the days I was told to come up. I think she is a free for all because she ultimately condone VALERIE'S back door practices there. There seems to be a good ole boy system up there that they think is iron clad. PLEASE DO YOUR HOMEWORK ON THIS PLACE. One day they will be caught up with and I hope it's soon. I wish I could be a mole worker in these places and get the bad people out of these positions that are meant for good workers. Do your due diligence about these people because nobody had anything good to say about them as workers.

I am a CNA at Southern Trace . I have also been a resident at Southern Trace . I had back surgery was left with no filling on my left side of my body where the hospital did not walk me after surgery per my doctors protocol. I called the administrator Nancy Brown, she immediately had me transferred to Southern Trace For therapy I can tell you I was back to work three months to the day when you are a resident you are angry family members are angry because this might’ve been preventable however, I was able to return to a normal life because of the care and my determination to be all. I could be the way I see it. You don’t use it you lose it I’ve always said that.
