If I could give this place a negative 1000 stars, I would. Nothing professional about this place. Truly, i've never seen so much neglect in my life and they take no accountability for any of it.

My dad was placed here in November 2024, and he passed mid January 2025. When he was checked in here, he was in good health minus his Parkinson's. Within a month, he developed fast growing pressure ulcers all over his legs and butt that were a result of him not being proper changed and treated enough. He then lost a significant amount of weight, and his ability to speak. It got to the point where he got very sick, and we were advised by the nurses on shift to take him to the ER that same day. As we made arrangements to get him to the ER, we were then stopped by the director of nursing, which looking back was them trying to save themselves from probably being reported by the hospital that we would have taken my dad to. We then got an outside opinion from a board certified doctor who confirmed these were stage 3 pressure ulcers, and considered this to be a "never event", which essentially means that they should have never occurred in the first place and were entirely preventable. My point is, this was all due to patient neglect, and it wasn't just my dad we saw levels of neglect happening to unfortunately.

I acknowledge this is an incredibly hard line of work to be in, but this should never have gotten as bad as it did if this facility was as upstanding as they claim to be. Especially for a place that "prides itself on its care". Im certain we aren't the first and wont be the last. When my sister and I met with the head administrator, Nicole, to talk about this and about getting some more specific care for my dad, she gaslit us by laughing in our face and asking if we "met with a psychologist" to diagnose my dad's ulcers and not an actual doctor. Implying that the second opinion that we got wasn't credible. It was clearly avoidant of how serious this was for an 86 year old man.

In the end, they pushed my dad to move to a sister facility in a very, very sick state because they wanted him off their hands. This new facility that we moved him to is where he ultimately died 1 week later. After everything though, I'm so glad he passed in a cleaner, more professional and caring facility than this hell hole of a place.

Anyway, to make a long and sad story short, don't place your people here if you have other options. Especially if you love them. I grew up in Marin and I'm just so shocked and disappointed that a place like this exists. My dad was my first aging parent experience and if we could have done things differently, we would have. That said, i know that not everyone has the means for better care (we didn't) so with that i would say, if you have to place your loved one here, PLEASE advocate for them. Please. In every step of the way. That will make the difference.

My final word is that these people don't care about the wellbeing of their vulnerable patients and place more value in operating as a business (speaking directly of the admins, not necessarily all the nurses). Example 1 being the way they reply to all bad yelp reviews doing damage control.

Sending anyone going through this situation love and light, and i promise things will eventually get better.

Professional Post Acute has experienced a remarkable transformation under new management. Having been at this facility for several years, I can confidently say the changes implemented by the new administrator have elevated it to a truly exceptional skilled nursing facility. The building has been freshly painted, and the flooring in common areas and resident rooms has been upgraded to hardwood.
The care provided here is now highly focused on customer service, ensuring that residents are both happy and well cared for. The improvements are evident in every aspect of the facility, making it a wonderful place for residents to live and receive care. I truly enjoy working here day in and day out !

This has been a great place for my short term rehab! Staff has been so nice and sweet to me. Residents are all good people and easy to make friends with. I enjoyed the activity program and playing bingo.

Recently visited the building undergoing renovations and was pleasantly surprised by the evident care from the new management towards residents and employees. Walking through, I noticed a vibrant community vibe with residents actively participating in various activities. Despite initial concerns from previous reviews, speaking with the new leadership on site quickly dispelled any worries. Looking forward to seeing the positive changes ahead!

A very close friend was sent to PPA following a stroke. While the staff is pleasant and do what they can, they are very understaffed and staff turn over is constant. If you have an issue that needs addressing and you finally manage to find the person who can help you, when you call or ask to see them for follow up they are gone with no replacement for weeks. If your loved one does end up here, you really need to visit frequently to make sure things like teeth brushing and diaper changing are happening. If the facility is closed to visitors you will never be able to speak to them by phone.

I spent 2 1/2 weeks here following shoulder surgery. I can't say enough about it. The care was very good, the staff very attentive. It was obvious to me that most everyone who works here seems to be happy to be here. It didn't take long for me to feel a part of the "family." The facility is easily navigable and kept very clean at all times. Thank you Professional Post Acute Center.

My sister, Florence was at this facility on and off for almost 7 months following 2 hospitalizations. Initially we were hesitant in agreeing to sending her to this facility due to the low ratings and because we live in SF and/or Daly City and commuting would be difficult. But at that time no other facility in SF or Daly City wanted to accept her, except Professional Post Acute, because her Medi-Care was running out (apparently Medi-Care pays a lot more than Medi-Cal, which my sister also has). We had no choice but to agree to let her go to Professional Post Acute. To our surprise, the staff there were extremely professional, compassionate, caring and attentive, not only to my sister's needs but to ours as well. They always greeted us with a smile and went above and beyond their call of duties to assist the patients and family members. They made my sister felt so much at home. If it weren't because of the commute and her nephrologist is at UCSF she would not want to leave this facility. To the administrators and all staff at PPA, thank you from the bottom of our hearts and keep up the excellent work!

This is the most horribly filthy medical facility I have ever seen. Smells like urine everywhere. They still have asbestos popcorn on ceilings. There are bugs and insects flying around. No AC, just industrial blowers in front of every other room. My tennis shoes stuck to the carpet. The bathroom was as unacceptable as her room FILTHY. Nothing is wiped down. A breeding for ground disease My best friends new robe went missing, her PJs, her money. AND THEY LET THE PATIENTS FEED THE CAYOTEs on the back patio. My best friend has open wounds on her head. I Thought Coyotes were sniffing out the wounded. These Cayote’s were out in plain site at 11AM. Patients were unattended while this animal was roaming. Todays head nurse was nice. Also, the beds are so old in this facility they are in need of new replacement hospital beds. I bet the county jail is more clean then this place. This is a medical facility caring for our sick friends and family and it’s Third world right here in MARIN COUNTY.
