I love this place, the case workers are Amazing and helpfully. Nursing is delivered at highest quality. I had my Dad here for two months. Highly recommended.

Very bad service. My dad was sent here after a stroke and the routines were missed and we kept having to remind people on what he needed on a daily basis. They also forgot to send the correct paper work when he needed his gi tube removed and got canceled. Now a few more days with the tube because someone forgot to send the papers. Very disappointed in management. Put 2 stars because not everyone is bad, a few people were very niceand helpful, but this place lacks good management.

I found the Trabucco hills nursing home staff to be courteous, caring, concerned, and attentive to the patients.
I was pleasantly surprised at the care my mom received, and how she responded from Physio Therapy down to the CNAs who showered and fed her attentively. Yes the facility certainly needs updating but ultimately it’s the supportive staff that creates the atmosphere so patients feel comfortable, cared for and a desire to stay.
Initially, i was put off by the the somewhat rundown condition of the rooms, tho’ on my frequent trips to see my mother at different times, I noticed remodeling going on to refurbish the facility. The daily care takes precedence for me; food was brought on time and was plentiful-the cnas took the trouble to feed my mum. So, in my book Dr. Ghosh, Eileen case mgr, Val & Amy nursing supervisors, Cheryl, Carmen, Giovanni, Julie, Kenya and Kimba as well as many others, receive full marks.

This nursing facility was a wonderful place for my mother to recover from her broken hip. The physical therapy and occupational therapy department is excellent. They were always taking my mom from her room giving her exercise and monitoring her progress. My mom even enjoyed the food! Especially cute is the little ice cream parlor that is open every day at 1 PM and the patients can go and enjoy a treat. They often would have live music on the holidays or for no occasion at all in the dining room. We enjoyed a country music band and a wonderful orchestra with violins and cellos. There is a calendar of daily events that the patients can attend and are encouraged to do so. I am so glad that I did not let the negative reviews on this site turn me away from this skilled nursing facility for my mom’s temporary rehab. Before placing my mom, I went there and saw it for myself and met with the staff. It was clean and cozy and had its own in-house physical and occupational therapy department. My mom even had a private room. I gave it a chance and I am very grateful that I did. My mom is back home now and misses the staff.

I want to take express my absolute gratitude to Katherine Young, Nurse and Case Manager for the Lake Forest Nursing Center. My dad was a patient here for 2 weeks. During this time I received daily updates and follow up from Kat...aka Katherine. She is passionate about her work and her patients and it shows. My dad was extremely happy during his stay and has expressed possibility for more long term care if needed. My experience with these facilities have not been good at all. Having handled my mother's care as well my experience of what quality care for a loved one should be was little to be desired. The importance of recognition for a job well done is so crucial to businesses such as this. So, again I thank Kat and the entire staff for making my dad comfortable and genuinely caring in his time of need. ❤

It took 2 weeks for them to find my mother's money. Now they are saying that I picked up my mom's things to her case worker, which is not true. They are so unorganized. I called one day and I told them my mom said she is having a hard time breathing and they need to check on her. The nurse said I just checked on her and she is fine. I was like no I just got off the phone with her and she said this to me. They checked on her and sent her to hospital. I should not have to ask twice for them to check on her! She came back from hospital after about a week. She was there for about 3 days and they weren't flushing her tubes connected to her kidneys and went septic. I came to visit and she was not able to talk or know I was there. The CNA said she had been like that since 6am and she didn't know what was wrong. I furious on their response and action. Since I was there they said they would call 911 since it seemed like an emergency. Glad I was there so they did their job and not just let mom lay there and die. Now we are waiting for them to find her things. They place also smells really bad unlike the new nursing home I have placed her in. Definitely do not go here. Oh yeah and they also gave my moms narcotics to my moms friend to give to my mom since they were too busy to do it themselves. So many violations and laws being broken here.

The WORST UNskilled Nursing facilty in California.
I am a registered nurse, so this review is based both on my professional opinion as well as a concerned family member who witnessed and experienced 10 days of unprofessionalism and neglect.

My mother would use her call bell after the first day of total knee replacement surgery to use the bathroom and wait at times OVER AN HOUR to get a response. It was so bad that she asked to start wearing depends because they were so unreliable and she didn’t want to soil the bed and have to lay in it for who knows how long! UNACCEPTABLE!

As for the nursing staff: My mothers first nights she had a nurse named Prince, I had to have a serious conversation with him because she had been admitted for 16-hours after surgery and had not received one pain pill! He claimed “they had not come from the pharmacy yet”. I’m an RN I know better. So, after I stood at the nurses station and had him call the pharmacy to check the status and also check the med cart…. the meds magically appeared! After that, I made sure to watch him give her pain meds/narcotics, and had her call me each time she asked for them! I also had to talk to the other daytime nurses every day about them not giving her pain medications when she asked for them.

Finally, Discharge was another disaster. The case manager Cassandra is irresponsible, unprofessional and incompetent in fulfilling her role. I can say such confidently because I am an RN Case Manager and I advocate for my patients. I would expect nothing less from this Case Management team!
First, my mother did not receive all her medications on discharge. She received the mostly used up blister packs they had on hand. We had to return TWICE to the facility to pick up all the medications.
Secondly, the case manager did not order any of the medical equipment (dme) my mother needed at home specifically: a shower bench, and a WHEELCHAIR! How is she supposed to get to her appointments after a total knee replacement?
Multiple phone calls to Cassandra AND to administration went unanswered!
Also, it has been a week and no return call still. I have been more than patient. This facility should be reported to OSHA and shut down!!

Completely unacceptable! Keep your family members away from this place!!!!

My mom stayed at Trabuco Hills Post Acute about 2 months ago recovering from her hip surgery. Very clean facility and from what i witnessed they treated patients well. The staff was always polite and responsive. I would definitely let my mom stay here again.
