My husband returned to this facility in November 2024 after complications from surgery. After having been in multiple facilities over the years, I was glad that he found this facility to his liking. The front desk woman was friendly and answered questions, the staff supportive and kind, and the rehab staff pushed him to achieve its goals. I have to commend the social worker (Jessica) who coordinated a team update with me and carefully planned his discharge. I felt like she heard what I said and understood my point of view. However, my husband lost 7 pairs of basketball shorts ( some with his name on them) even though we had requested not to have his clothes go through the laundry as we would solely be responsible for that task. I would advise those families wanting to oversee their loved one’s laundry to put up a sign on the clothing cabinet so it will be very clear that they will do the laundry. Finally there is limited lighting in the main parking lot at night, so it’s a very dark and ominous walk to the facility. I hope more will be installed to keep everyone more safe and secure at night. Our biggest compliment to this facility is knowing this is where my husband will continue to go if a skilled nursing facility is required post-hospitalization.

My dad has been there due to him having a pacemaker put in and needed physical therapy and occupational therapy. The nursing staff and CNAs are amazing. The ones that have been taking care of my dad all speak Spanish and can communicate very well with him and he’s able to let them know his needs. They are very attentive to all the patients and friendly. The only thing is the clothes I took for him is missing even if his name is visibly written on them. It’s better to let them know that the family will do laundry to avoid it getting lost. I would definitely recommend the facility for anyone who needs to get rehabilitation.

My brother has been placed in Fountain Valley Post Acute facility after he had surgery on his broken hip. Let me start by saying my brother is 58 yrs old with Down Syndrome and because he is so special and sweet, I am VERY picky with his care. If anything isn’t going great, I will be the first one saying something! However, so far I am very impressed with everything here. The facility is clean and the staff have been wonderful! They really seem to care about the patients and are going above and beyond for my brother as well as all the patients! It sure helps me to rest easy when I leave here, knowing he is in good hands. Thank you so much everyone!

They do a great job here! The go the extra mile to take care of the guests. (Patients!) even setting up a candlelight dinner with live music and great food! Very happy with this facility for my little sister!

I was recently involved in assisting a family who's mother had a stroke and was moved to FV Post Acute after a few days in the hospital. I can't say enough about the great treatment my friend received, and how thorough the communications were about her care and case management. A BIG shout out to Mireya, the social service coordinator, who was so very, very helpful as she guided us through the process of admission and getting a meeting with the Nurse Case Manager April and the PT Mary. They gave us all the time we needed to explain my friend's current status and we never felt rushed but were given plenty of time to ask all the questions we had. The discharge was handled equally well ... we were informed how it would happen and they took care of EVERYTHING, including ordering the medical equipment to be delivered to her new home at a Board & Care. What a relief that was! Thank you, Post Acute, not only for hiring great staff but for providing what was needed at a stressful time for the family.

Everyone is nice at Fountain Valley post acute. The nurses and the people in green are very patient with my mom and treat her well. The meals are nutritious and good. My mom has changed positively since her admission. I would like to thank all the personnel for their good work and compassion; it’s not easy to care for the elderly.
