I don't know how this joke of a nursing facility has a 4 star rating.If I was able to give a negative 10 star review I would, but one star is the lowest I can give. I am actively seeking another facility for my mom. Forest Park is worse then a roach motel. The menus choice have got to be the worst I have ever seen. There is nothing nutritious about the the slop that gets served to the residents of Forest Park. The food is so bad , the staff won't even eat the food. You walk in the building and you are welcomed with a fresh aroma of urine and feces.The building is absolutely filthy.There are a some staff members that actually do care about the residents but there are more that care about the one thing, money.They care so little about the residents there that they have received 2 I.J. tags. In case you don't know what they are they immediate jeopardy tags which is the worst thing a facility can receive from the department of health.The department of health is there on a weekly basis. If u truly have a heart and care about your loved ones, DO NOT admit your loved ones here .

They refuse to let me talk to my Mom on the phone. I live in another state. Reported it to the state once & was told they were in the wrong & the administrator at the time bent over backwards to fix the issue. Mom doesn’t have a phone in her room. No one will take a cordless phone into her. I have missed wishing her Merry Christmas & Happy Birthday because the facility does not show compassion or caring. They only want to blame me & tell me to get her a cell phone. Wrong! What do they do for those that are unable to work a cell phone? I would never recommend this facility! Their website is the total opposite of what you will get from them! It’s not ok to treat the elderly like this & keep them from talking to their families. Also in her old room she had no phone & after I complained the last time this happened they put a phone in her room & told me she should have had one. So she moved rooms & no longer has it. Crazy!

My mother was admitted here on October 1 2017. The first day I visited all was good. Nurses came in and checked on Mom.

While I was visiting her the second time, a male nurse came into her room with a needle full of medicine tucked behind his ear like he was Mr macho man. He gave moms neighbor the needle behind his ear an injection with that needle!

Went by another day and moms feeding tube was pulled out and stuff was oozing out all over her and all over the bed. I marched down to the desk and gave a piece of my mind to the nurses about it. They immediately went to her room and fixed the feeding tube and had to clean her up and change her gown and sheets.

The next time I visited she was in a chair by the nurses desk because she was trembling very badly. They brought her back to her room and put her in bed.

This place is not a 5 star accommodation either. It smelled dirty when we walked in. I was in the process of finding another nursing home to move her to, but she had a severe seizure and passed away on October 9, 2017

It was a Pleasant visit and atmosphere . My father was happy and said he liked it and that’s all I needed to hear . He was clean and warm and very comfortable . A big difference from the last nursing facility he was in .

The staff was very friendly and welcoming! They seemed to truly care for the residents…. There were several staff members observing and interacting with those in memory care.

The facility seemed very clean- no urine smell! It was smaller which was another plus. It was not a top notch facility but was clean and comfortable. I felt satisfied leaving my husband with Parkinson’s/dementia there.
