Terrible place. My father fell three times while there. He became altered mentally the beginning of the third week he was there, after testing, I was told it was due to a UTI. A week later I had to have them call an ambulance because he got worse then he died 3 days later. There were a couple of good employees but for the most part my father never got his meds on time. Repeatedly would remind them his back was broken and he needed some Tylenol but they would never give for hours. Female Director assistant sat on the chair in the lobby talking to two other employees. When I tried to get my dad outside with one hand and a hip on the WC, because I had his FF and Shake in the other, they all just sat there until I asked the man to open the door for me. Then did not offer to help me push him out to the table. Terrible customer service.

I am givingt this one star solely based on the fact that this facility has some nice people working there, but the facility itself is severely lacking in many ways. It is beyond cramped, inadequate in accommodations, and does not lend itself to a healing environment. It is understaffed and does not have adequate safety measures (such as bed alarms) in place. My mother's roommate, who was there because of a broken hip, crawled out of her bed falling onto the floor ten times while I was there and would have just remained there unattended had I not noticed and called for help. Food , which was cold each time it was delivered, was left for my mother to cut herself, which was impossible for her to do. The staff giving the medication was sometimes unaware of what they were giving - some are not licensed personnel. One staff member came in and told my mother she was giving her heart medication - when I questioned her and she told me the name of the medication, I recognized it was not even a heart medication, but the staff member did not know the difference. Makes one question how many medications are wrongly administered. They do have some licensed staff, but they are understaffed, leading to delegation of critical tasks to unlicensed, and apparently untrained personnel. The rooms are impossibly small, outdated and dull. If there is more than one visitor, you are out of luck as there just isn't space left. Again - the staff are probably doing the best they can, but if you can find any alternative for your loved one, please avoid putting them in this facility. It is depressing, inadequately maintained, has severe concerns for basic quality measures of physical safety, lack of healing environment, and inappropriate use of staff for daily tasks.

If you care about your loved one Please don’t send them here. It’s not the employees fault they are doing what they can. This place has been so understaffed for years. It was wonderful when it was Golden Living, Diversicare took over and the employees left. The corporation doesn’t care about the patients or employees. If you don’t believe me go by there especially at night. Call lights are on and not enough staff to help these poor people. Sometimes one aide to each hallway. Our elderly people are being ignored and left behind. If I could give them a zero I would.

My mother was a resident here until her passing. They always were so sweet to her and took great care of her. Having the hospital next door was also a blessing. Such a comfort to know if she needed medical assistance it was mere footsteps away. My Aunt is currently a resident and she is receiving the same excellent care. It was a huge concern putting our family in a care facility with some of the horror stories you hear, but not at Diversicare. They truly care for and about the residents at their facility.

The team at Diversicare of Boaz takes care of their patients and residents like family. The therapy team is one of the strongest around and makes sure that they do everything they can to help their patients and residents get to their individual next level of care.

Wonderful staff members that are hardworking and dedicated to their jobs. A place that you can say really is a home away from home when you need to have a family member taken care of for any kind of need. Their doors are always open to anyone looking for employment or care and the staff is always their absolute best to provide the gold standard of care to not only the patients but to the family members as well.

Both my mother and my aunt have been or still are residents at the Boaz location. Excellent staff and nurses! Go above and beyond to care for their patients. Never one minute do I worry about my Aunts caregivers. I have already recommended your place multiple times and will continue to do so!

This is the worst place I have ever seen in my life, they need to do something about this place, people check in on your family more you never know how they are treated,the state needs to come in and check them out ?
Cause they are a lot that is going on
