I elected to update this review based on the following events with this provider, especially when someone in my PCPs Office assigned them to me
I will be following up with the appropriate agencies, both Federial Medicare and State. This home care provider should have their license revoked, and even their own employees advise they are short staffed and some patients are not receiving the care they should.
They were to start one month but never showed up until 6 weeks later. No call, no apology, nothing. Then the OT Lead a woman showed up for three vists, one of which was for an evaluation for Elec. Wheelchair, one more visit, and she was gone. Took her 2 weeks to submit paperwork
Even the device provider acknowledged they were lax in completing and filing paperwork. However what do you expec5 when for all of Washington and part of Green County you have 2 OT full time therapists. Oh and Lymphadema OT is knowledgeable amd n8ce individual BUT showed up once. Missed next apt. Never saw him again. Now as proof I have phone records. From the 5th of the starting month until I fired Concordia on 21st of following month 50 days total, this OT individual called 6 times, left one message. But claims I never returned their call. REALLY!!!!
PT Technicians were good but they stopped d3spite promises by Concordia they would restart, weeks go by nothing. Make promises they will do better. Branch manager never returns a call. I also reviewed charges with UPMC now will follow up with Medicare as wrll in what was submitted.

My mother is receiving home health care with nurses PT and OT. They have been so wonderful in every way! Highly recommend them.

Chris came for weekly physical therapy, he was very professional and helped my recovery so much. Always pleasant and kind, called ahead before each visit. I would recommend home PT by Chris to everyone.

At first we were a little frustrated about the scheduling and my mothers lack of motivation to rehab after a bad fall and broken hip. But we worked out a schedule with OT Carolyn, and PT Kaylene and my mother is now a very independent person who walks very well and does most adls herself. This process doesnt happen overnight but it did happen. We are very thankful to our PT and OT. They are patient, and very knowlegable. My mother formed bonds with them. We are very grateful for their assistance and would highly recommend Concordia.

These people suck with home care, said they'd work around our schedule, that's not true, and now we haven't heard from them in 2 weeks, the nurses they sent, talked and told you their life story, we could care less, come in, do your job, and hit the road!

They do not contact you until the day before about who is going to your house, and they do not check notes about the places they are going if you need any special accomodations. Absolutely unacceptable for home health services.
