Currently I am the resident at Jacob Healthcare Center. I do really appreciate the care from the Nursing and Rehab Staff. They are all trying their best to help. The facility is clean and nice. If it wasn't for the staff I would not have regained my strength and been able to return to my full strength.

Please do not leave your loved ones at this facility. Staff is not helpful at all, no one has answers, and overall poor everything. My mom was sent for physical therapy and would beg us to take her out because they were neglecting her and not giving her the care she needed for recovery. Unfortunately she died only a week later in n their care. It only takes one visit to realize this place is a depressing and inhumane place to leave a loved one.

Currently, I am a resident at Jacobs and wanted to leave my initial impressions. I will update this soon discharge, as well.

First and foremost, I am always an advocate in my own care, so very probably that means a pain in the arse, lol. But it also means that I am aware of what is truly important. And that means people.

Without fail, every person I have had contact with has had a genuine desire to assist in any way possible. And with a smile and a sincerity that is GENUINE, and that is something that cannot be bought, fake or ignored. The staff here has it in abundance. A special shout out to my CNA, Alyssa, Concierge Manager, Susie, Social Services Manager, Noemi and Dietary Supervisor, Ruby. These 4 women, got the worst of my complaints, saw me through my roller coaster of emotions of being separated from my precious dog , frustration of having little control over my surroundings and of going home safely. All while trying not to take over being the cook, doctor and facility coordinator! 🙃

What they did do was hold my hand, listen to my concerns, and I’ll be darned if they didn’t solve every one of my concerns! From helping me find a way to get home sooner, care for my dog in the interim, fix my glasses so I could see, giving me the fuel my body need to heal, all while feeding my soul. (Sounds so danged corny, but true!)

So for now I am giving them 4 stars, but I am confident that when I update this upon discharge that it will be even higher. Thanks to all of these special people! Until then… I highly recommend. 🤗

I’m not sure how they rate themself 5 stars honestly. Nurse slack a lot, they are only caring when a visiter are around behind close doors they don’t care. No proper equipments for therapy. No dissent food. Anybody can just walk in especially at night no guard or security. I feel bad for the elderly who actually live there. I’ve seen nurses ignore them for a long period of time. Most of the time it looked like they were short staff or nowhere to be found(especially in afternoon or night) Most of the nurse seemed like they didn’t know what they were doing or just felt like not doing it. Nurses get offended or upset when they are continually asked for help.. ….I’d visited my mom everyday, I had patients asked me for help becuase nurses couldn’t be found. Some patients are even scared to mention anything anymore so they won’t be mistreated Any way. It so sad. I honestly could say 1/10 are professionally caring and doing their job. It’s just devastating how inhumane they can be sometimes.

My 93-year-old sister was in an accident where she ended up with a fractured hip. After the hospital did a great job taking care of her, they helped us find a Rehab Center.

They recommended Jacob Healthcare Center and after reading a couple of the 1-star reviews, I was hesitant but relied on the overall 4.2 from Google and the 5 Stars from the Medicare website.

We arrived in the evening and were greeted by a few friendly faces that quickly put us at ease. There was a hiccup overnight when some paperwork, a covid and TB test, that probably could have waited until the next morning were done.

The first morning however, there was a steady stream of administrators, social workers, caregivers, nurses, CNAs, a doctor and specialists from every department in the facility that stopped in to check on what my sister needed or to advise us what to expect during her stay at Jacob.

By the time I had to return to Arizona a few days later I was so relieved and pleased to have found Jacob for my sister. I call her daily and she sounds stronger and happier with each passing day - hoping to hear she'll be ready to return home in the next week or two, but I am sure she will miss everyone...even the wonderful team in PT and OT!

For those reading this in a similar situation, where your loved one's world has just come crashing in on them - you can rest assured that they will be well cared for here.

I had a bad accident, compound fracture, other broken bones and injuries. The hospital released me and recommended a habilitation type treatment center. I'm glad I chose Jacobs. Their knowledgeable staff and facilities helped me too be able to return home confidential. I had PT and OT to help me deal with my condition. The staff was friendly and easy to work with. They had many activities to help me pass the time as my body healed and became stronger. I highly recommend this facility to anyone in need of the services they offer

This is the worse place to be nurses don't do their job when u ask for pain pill they give u sleeping pill. And u never see a doctor here they claim there is but never seen a doctor I been here over a month. They can't even get you to a docyors appointment to where u have to reschedule. .
