Zero stars!!!! This is the absolute worst assisted living facility I’ve ever encountered. PLEASE, I beg you, do NOT leave your loved ones here. The conditions are beyond appalling this place should be shut down immediately. The facility reeks of filth, as if it has never been cleaned or sanitized. The staff is poorly trained (if trained at all) and seem indifferent to the suffering around them. Patients are screaming in distress, and it’s heartbreaking to witness. The environment is chaotic, unsanitary, and outright inhumane. Someone, please, FIX IT, JESUS! This is not a place of care; it’s a nightmare.

If you care for your family, look elsewhere. This place is unacceptable!

I have so many wonderful things to say about San Miguel Villa! My aunt has been to many rehab facilities in the area and our experience here has been phenomenal. Yes, they are still a skilled nursing home so I think it's unfair to expect a Ritz-Carlton level of service like others who have left reviews. BUT you can tell how genuine and honest this team is, which is a game-changer in this type of care. The Social Services team is incredible and works so hard. The administrative staff is easy to talk to and attentive. And the nurses are the cutest and kindest group of people I've ever seen!

I actually visited San Miguel Villa nearly two years ago and it feels like a completely new facility now, which is why we gave it a shot! It looks like they're currently having some remodel work done. Although they still have a lot to finish, it looks really good. Their lobby is beautiful and my aunt's room had brand new floors installed and was well prepped when she arrived. Within about an hour, she had a warm dinner ready for her and the nurses were happy to see her. Honestly, this all scared me a little because I was concerned that this was only a first-impression amount of effort. Now that my aunt has recovered and is preparing to leave the building, I can confidently say that this has all been a great experience.

For the sake of being unbiased, I will admit that there were about two days where one of the patients kept smoking in a room down the hall, which was my only complaint. I know this a complaint more about a resident rather than the facility. I do know facility manager talked to this person several times about the issue and the patient ended up leaving soon after.

In the end, I would highly recommend this rehab facility to anyone. I wish I could give my personal contact information for anyone to call me and talk more about it.

My Brother John(JD) was there about 10 years. For the most part there services were great, Especially Linda, one of the Head Nurses. Not to mention many others.
But on the other hand we had our problems. Some of the people that worked there should have not been in that type of
Business. They were mean and
cruel.
I know it's not an easy job. But come on, have a little compassion, empathy, kindness,
and love.

I went to see San Miguel Villa for my aunt rehab. The place was clean and has friendly personnel. During the time my aunt was there all the Staff members were incredible. Always smiling, informative and detailed. I can not say enough about the care she received there. Honestly have no complaints.
I highly recommend San Miguel Villa skilled nursing when you need skilled nursing and rehabilitation.

As the Resident Council President of the facility, I'm a huge advocate for making this facility feel like home for most. I've been here for roughly 4 years and most places have their ups and down but its pretty managable here. I get to know patients who are here for rehab and most of them seem quite satisfied with the care. I know this isnt typically a place people want to come here to recover, but the staff here makes their stay bearable. They really go above and beyond. Staff on the long term side feel like family especially when they know your routine.

Mom has been a resident at San Miguel Villa for a few months. This experience has been a real challenge for her. The staff have worked diligently to make mom feel both comfortable and safe. I appreciate how my complaints and concerns are handled. Most of the workers are professional and have gone out of their way to provide excellent care. The facility can definitely use more assistance with answering the phones. It becomes so frustrating at times to not be able to talk to your loved one or get an update from the nurse.

I am giving San Miguel Villa 0 star because we had the most horrible experience visiting my dad on his first day in that depressing place. My dad was transferred around 2:30pm to this facility, we don’t know what time he got there. But, as soon as I got to this place around 6pm with my mom and my husband, we witnessed my poor dad trying to move on his own and asking for help when he couldn’t really move because he had just gotten a pacemaker on his right side and has a blood clot on his left side. My dad’s mobility is very limited since he had been in the hospital for a month already and the social worker recommended an SNF to receive PT so he could get stronger and come home. Social worker did not give us much option on facilities near us so we opted for San Miguel because it had a 5 star and it’s easier for us to go to Concord. However, now we regret it and do not recommend this place to anyone, my dad was very depressed as I arrived he almost cried telling us that they had not fed him, he was cold and they only gave him a very light blanket, he was shivering, and he was asking for help to move around the bed and they ignored him, someone even dare to tell him that his daughter was not going to visit him without calling us to find out anything. He had not been fed or gotten anything to drink at all and he was super hungry. I asked for answers to the lady in charge that night and she tried to explain why he had not received his food but I said that was no excuse. He was hungry, then she came back apologizing to me saying it was her fault she had forgotten to put the diet recipe into the kitchen. I said that was unacceptable to me! My mom cried just by seeing him asking for help. We decided to get him out that night and we couldn’t even get a list of the medications he was getting. This experience has been the most horrible ever. My 71-year-old dad did not deserve these kind of treatment after surviving covid and going through so much this month! Please do not trust San Miguel Villa!

Hello, My mom has been here at San Miguel Villa in Concord for about 5 months. Can't say enough for the kind staff: Cna and Nurses that have always made my mom comfortable and happy. Long time staff like: Baby, Rena, Pedal, Maria, (charge nurse) Maria Morales, Lvn Alecia, and many others.. Everyone always helpful and asks if I need anything. Can't say enough. If you are looking for a care facility this is the place to go. They're not all perfect. But this place is pretty close. Thanks San Miguel villa keep up the good work!!
