I've been here for 3 days now, and I'm impressed with how friendly the staff is. They seem to care about my recovery. The facility has a nice courtyard to sit and enjoy.

EDIT: When I left this place they never reported to the Social Security Office that I was discharged and I had to jump through hoops getting my disability payment back to what is was suppose to be.

My sister has been living at Boulder Creek Post Acute for four years now. I see how kind and wonderful nurses and CNA's are towards her, and when I have a request or concern of any kind they are quick to respond. They don't have it easy there. There are a lot of people living there permanently or temporarily and they are constantly working, taking care of one person after another, but they do this with a smile on their face. These folks are givers and their jobs aren't easy. I am so thankful my sister is there because I know they are taking good care of her and she's happy. Thank you, Boulder Creek.

I want to extend my most sincere thanks to Boulder Creek for all they did for my mom and the special circumstances of her case, being that she lives out of the country. From the moment I stepped into the facility I felt welcome. The high level of commitment they have for their patients is obvious. I knew mom would be in great hands. They all went above and beyond to accommodate our special situation (mom needed post hip surgery rehab).

Even though my mom only stayed there overnight (for personal reasons, it had absolutely nothing to do with Boulder Creek), they still helped us navigate the post surgery system, sending her meds to my home. They even sent her flowers with a beautiful, heartfelt message.

If your insurance forces you to go to this facility and you have no choice that is one thing, but if you have a choice and /or paying out of pocket, please don't do it.

My mom checked into Boulder Creek on Wednesday March 17 at about 3pm for 2 weeks minimum. A nurse Erica called to get verbal consent for treatment. It was 1 nurse and NOT the standard 2 Nurses that required for verbal consent. She also told me to bring my mom’s advance directive and clothes on Thursday before 4pm and I would also be able to see my mom through the window. I get there right before 4pm (which coincidentally is shift changing time). Lita, the activities coordinator told me i shouldn't be there, the nurse Erica was wrong, i needed an appointment, but they would allow me to see my mom this one time.

I hear nothing else from anyone in the boulder creek facility.
My mom told me on that her check-in day a male staff asked her how she was able to get in the facility. He also was rough with her, but we let it pass. Sunday March 21 the same male staff was rough and rude with my mother. He told her she was not the only patient in the facility and left her in a soiled diaper for over 45 min. Monday March 22, my mother told her dialysis social worker what was going on and they reported this as elder abuse. It was reported to the facility, the state and the sheriff.

Monday afternoon about 4pm the director Dean and a nurse called to advise me that they got a claim and was investigating the claim to fullest and anything i needed they were there to support us. Tuesday March 23rd i got a call from a case manager that they wanted to have a team meeting to welcome my mom to the facility, introduce me to everyone providing care for her and to discuss my mom’s care and make sure we all had a plan of care for her success. We had the phone meeting the following day March 24 and everyone was just so nice and wanting the best for my mom. They had tentative scheduled for different activities planned.

Wed night, after the team meeting, my mom calls me and says she was told she was being discharging Saturday. Thursday March 25, Veronica the case manager calls me and says, my mom's insurance was no longer covering her stay after Friday March 26, and we needed to pick my mom up on Saturday before 4pm. I told Veronica we were already informed of this info on Wednesday after the Team call. Veronica was adamant that she was the point of contact and she JUST got that info right before she called me on Thursday.

I contacted my mom’s insurance on Friday but didn't get a call back until Monday March 29 but they said they did NOT know my mom was being released and they did NOT stop paying for her stay. The Insurance said that 9 days was not a time frame they would even stop paying. The insurance contacted the Boulder Creek facility, and no one had a valid reason as to why my mother was released. My mom had to go back to the hospital (checked in) on Mondays 4/1 because she was still having complications.

Also to add they released my mom without her oxygen tank that she was admitted with and it took hours before someone was able respond to us. I called the facility and was told they were "too busy" to talk to me about the tank. I texted the director Dean, who 1st said my mom wasn't on oxygen, 2. Asked if it was really our tank. And 3. Finally said he would look into it. Then I get a call from Jane who said they found it... But when we went to pick it up, they misplaced it, again.

I have been a patient at Boulder Creek Post Acute Nursing Home for over 2 months for physical therapy. The Physical Therapy & Occupational Therapy teams and the nursing staff at Boulder Creek are very caring, considerate, and kind. They are always attentive to my needs. The entire staff of this hospital is exceptional.

The Boulder Creek PT team designed a therapy program to meet my specific needs. Each day, Clarissa (the Director of Rehabilitation) and Alma (my primary therapist) tailor my therapy regimen based upon prior days performance, my present physical condition, and goals. My health issues require constant monitoring and adjustments, and I have complete confidence in their management and direction of my activities.

I am very appreciative of everything the Boulder Creek staff has done for me. I recommend this facility without reservation.

I wanted to take the time to write a review regarding the staff at Boulder Creek. Lita, Marcella, Cindy, and May meet regularly to plan care for my mother who suffered a stroke in 2019. My mom has a rough history and most people would just write my mom off. However the staff at Boulder Creek treat her with respect and dignity and provide excellent care for her. She is actually eating now and maintaining weight. I know things could not have been easy for the staff at Boulder creek especially during the pandemic but through it all they maintained their professionalism and great standard of care. I want to thank them from the bottom of my heart.

My brother has been in this facility for about 5 months now. He isn't an easy patient and is not compliant for the most part, so I know the nurses and CNN's haven't had an easy time of it. I will keep that in mind while I write this review. Sometimes it has been hard to get through by phone to talk with him. I am all he has for family, so it is important to him that I be able to talk with him everyday. I don't live in the San Diego area so the phone is our primary way to keep in touch. That has been a problem at times, so that is why I am giving Boulder Creek 4 stars instead of 5. I will say that his social worker, Henry Huynh deserves 5 stars plus. I have always been able to get ahold of him and he has always answered all of my questions. He usually talks with my brother daily and keeps me posted as to how he is doing. Henry even gave my his personal cell number, which I have called numerous times. I can't say enough good things about Henry and the service he provides. Boulder Creek better hold on to him, he's a gem!

Been here a little over a year.I have nothing good to say about this place. I thout it was bad when I frist got here but it has just got worse. The c&a that seems to care have got to the point of being like the rest.They or here for a check The building is run down and getting worse. The roof leaks in lots of places. Dinning room is closed cause of leaks.They say they are getting istaments to get it fixed but I have not seen any one doing that.floor tiles in my room they used to replace but now they just let go.the pplace is not keep kleen and smell must of the time.The food taste like yuck must of the time.They do your Landry but good luck on getting your clothes backbackwheenthey break theyto replac
